You didn't notice it because you're accustomed to seeing "Yamazaki" in the context of H!P.

And you're accustomed to seeing "Yamazaki" in the context of H!P because there are two of them right now: Mei and Yuhane.

If that were not true, it's more likely that you would have noticed it.

It's the difference between thinking "There's no one named 'Yamazaki' in H!P" and thinking "I know there are members named 'Yamazaki' in H!P but she's not one of them". The latter requires quite a bit more mental effort than you normally spend on watching the opening credits of a television show.

If you put it that way it seems true
But in my perspective it really is just me not being observant on an obvious mistake regardless if H!P had members named Yamazaki or not. It happens

Idol Mou Icchou ended but the co-host of the show, Sawaguchi Keiko, started a spin-off corner some time ago called Sawaguchi Keiko no Kininaru Mou Iccho on the same channel. She mostly talks about (small and big) Hello! Project news and be an wota.

In the most recent episode, she had a conversation with the produce from Space Shower TV who has been producing Hello! Project and other idol shows for the network.

^^ Sotsuyou shashin is one of my fave Yuming songs. It was touching when Nanbara-san came out and sang the verse "Ano koro no ikikata wo anata wa wasurenaide (Don't you forget the way we lived in these times)" to her. Kamiko doesn't have the same husky-bell vocal quality of Matsutoya but the way she sings is similarly unadorned and honest, especially from her younger years.
